  • Note: You must report any changes in your name, address, email, phone number, and employment to OIA within 10 days. Also you must check in with OIA everry 6 months during STEM OPT.

    Complete the I-983 evaluation form with your employer at 12 and 24 months (or earlier if changing employers, visa status, or moving to another degree).

  • To apply for the 24-month STEM OPT, you will need to follow the following steps:

    1. Verify that you have maintained your F-1 status:
      1. You are up-to-date on your OPT reporting,
      2. Your 12 months OPT has not expired,
      3. You have not surpassed the unemployment days allowed, and
      4. Your STEM OPT employment will be STEM eligible.
    2. Complete the remainder of this form.
    3. Submit the completed I-983.
    4. Submit your most recent passport, visa, and I-20 copy.
    5. Submit your most recen I-94 and EAD Card.
    6. Complete the I-765 including:
      1. Question 27. Answer with the code (c) (3) (C).
      2. Your degree name and CIP code.
      3. Employer's name as listed in E-Verify.
      4. Employer's E-Verify Company Identification Number or a Valid E-Verify Client Company Identification Number.
    7. Submit the I-765 with supplemental documents to USCIS no later than 60 days from the issue date of the STEM OPT I-20 and before your EAD expires.
